At Florida Family Primary Care Centers of Tampa, primary care isn't transactional. It's a partnership. Our board-certified family medicine physicians and nurse practitioners serve patients from infancy through older adulthood, providing the continuity that leads to genuinely better health outcomes.

We hold the NCQA Patient-Centered Medical Home (PCMH) certification, a national standard that ensures your care is proactive, well-coordinated, and built around you as a whole person, not just a presenting condition.

Call (813) 284-7903 or submit our online booking form. New patients and same-day sick visits welcome. We'll confirm within one business day.

Complete Your Intake Forms

Arrive 15 minutes early for your first visit to complete patient intake paperwork. Bring your insurance card, photo ID, and a list of current medications.

Meet Your Care Team

Your physician will conduct a thorough evaluation, review your health history, and discuss your concerns with no rush. This is your time.

Receive Your Care Plan

Before you leave, your doctor will walk you through a personalized care plan, including any follow-up tests, specialist referrals, or medication changes.

Ongoing Coordination

Between visits, our team monitors your care plan, reaches out for follow-ups, and coordinates with specialists, keeping you healthy, not just treated.

Who We Serve

Our primary care services are open to patients of all ages, infants, children, adults, and seniors. We accept new patients and welcome families where every generation can see their doctor at the same practice.

We accept Medicare, Medicaid, and most private insurance plans including Humana, Aetna, United Healthcare, Simply Healthcare, Molina Healthcare, CarePlus, and Sunshine Health. Self-pay and cash patients are also welcome, call for pricing details.

All services are available in both English and Spanish.
